Notice of the Trade Union of Chongqing Yingtianhui Chlor-Alkali Chemical Co., Ltd. on Carrying out Mutual Assistance and Guarantee Activities for Employees

公司全体职工：
根据中共中央、国务院《关于深化医药卫生体制改革的意见》中“鼓励工会等社团开展各种形式的医疗互助活动”和全总提出的“加强职工互助保障体系建设”的要求，为大力弘扬工人阶级团结友爱精神，推动职工互助保障事业，不断增强广大职工的保障水平，增强职工抵御风险的能力，缓解职工因首次确诊患上本活动所列疾病导致医疗费用支出增加和收入减少带来的经济负担，现公司特为全体职工购买职工互助保险，标准为每人30元（其中由市总工会补贴10元，我公司补贴10元，个人缴纳会费10元），本活动按照自愿原则，请各基层工会按10元/人会费收取，并于2016年5月23日之前统一交至公司工会出纳殷英处。
一、职工互助保障活动基本内容
参加本活动后，在互助保障有效期内会员首次确诊患有本活动所列的25类重大疾病的一种或多种时，会员可按照本活动有关规定领取互助金，用于缓解家庭经济困难。
二、参加互助保障活动的条件
凡我公司在职职工（含正式职工、合同制职工）
三、参加本活动的规定
1.参加本活动会费标准为每人30元，交纳会费后互助保障期在约定时间统一生效，一经生效中途不得退出本活动。
2.本活动保障期为一年，期满续保另办手续。保障期满后，无论会员是否已享受互助金待遇，所交纳会费不再返还。
3.各基层工会需提供参加本活动的会员名单，内容包括姓名、性别、身份证号码、手机号码等信息。（电子版）
4.参加本活动的会员在约定生效的互助保障期开始之日起，无观察期，收费后24小时内即刻生效。首次确诊时间以病例检验报告诊断日期为准。
四、参加本活动的待遇和相关规定
1.重大疾病保障待遇
在本活动生效后，会员首次确诊患有25类重大疾病的一种或多种时，可以一次性领取重大疾病互助金10000元（每份），本期重大疾病保障待遇终止。
2.本活动所指的重大疾病包括以下25类：
急性心肌梗塞、冠状动脉搭桥术、恶性肿瘤、终末期肾病、重大器官移植术或造血干细胞移植术、白血病、良性脑肿瘤、严重烧烫伤、瘫痪、多个肢体缺失、严重运动神经元病、双目失明、语言能力丧失、重症帕金森病、严重阿尔茨海默病、心脏瓣膜移植术、系统性红斑狼疮、急性或亚急性重症肝炎、慢性肝功能衰竭失代偿期、严重重症肌无力、严重多发性硬化症、深度昏迷、双耳失聪、严重原发性肺动脉高压、脑炎后遗症或脑膜炎后遗症
五、下列原因，会员不享受本活动的保障待遇
1.战争、军事行动、暴动、恐怖活动或者其他类似武装叛乱期间。
2.不可抗力的自然灾害。
3.违法犯罪行为，从事违法、犯罪活动期间或者被依法拘留、服刑期间。
4.故意行为，挑衅而导致的打斗、被袭击或被谋杀。
5.故意隐瞒、伪造或篡改病史、病历以及其他欺骗行为。
6.酗酒或者受酒精、毒品、管制药品影响。
7.酒后驾驶、无有效驾驶执照驾驶、驾驶无有效行驶证或者驾驶与驾照不符的机动交通工具。
8.医疗事故导致的.。
9.非认可的医疗机构。
10.会员在参加本活动前已经或曾经患有本活动所列疾病的任何一种或多种，或由其他疾病转移致使会员患有本活动所列疾病。
11.医院误诊。
12.工伤、生育、职业病、由责任方承担的或者其他非疾病原因导致的。
六、互助金的受领人
重大疾病互助金由会员本人受领。
七、互助金申领
会员自首次确诊患有25类重大疾病之日起，应在5日内告知公司工会管理人员以便进行调查以及手续申报。
八、其他约定事项
1.本活动所指的重大疾病按照国家有关疾病诊断标准判定。
2.为维护全体会员权益，本活动随社会经济发展状况及国家有关政策变化将进行适当的调整。
3.对本活动执行中有关内容发生争议，由中国职工保险互助会专家委员会进行最终裁定。
